The above three are mandatory, rest are optional if u score in those categories, then ur chances are brighter. 4) Age (extra points for younger people). 5) UK experience (if u have worked one or more years in UK in last 5 years then u get extra points). Yes u can directly apply it (u dont need anyone to represent you). Rest of the procedure is described in full detail in the website I have printed below.
